1、進口非特殊用途化妝品備案申請表;

2、產(chǎn)品配方；

3、產(chǎn)品質(zhì)量標(biāo)準(zhǔn);

4、經(jīng)衛(wèi)生部認(rèn)定的檢驗機構(gòu)出具的檢驗報告及相關(guān)資料，按下列順序排列：A. 檢驗申請;B. 檢驗受理通知書;C. 產(chǎn)品說明書;D. 衛(wèi)生學(xué)(微生物、理化)檢驗報告;D. 毒理學(xué)安全性檢驗報告；

5、產(chǎn)品原包裝(含產(chǎn)品標(biāo)簽)。擬專為中國市場設(shè)計包裝上市的，需同時提供產(chǎn)品設(shè)計包裝(含產(chǎn)品標(biāo)簽);

6、產(chǎn)品在生產(chǎn)國(地區(qū))或原產(chǎn)國(地區(qū))允許生產(chǎn)銷售的Z明文件;

7、進口化妝品報關(guān)需提供其中文標(biāo)簽、貨物入境檢驗檢疫通關(guān)單、產(chǎn)品成分表、contract、invoice、裝箱單和報檢、報關(guān)委托書等手續(xù)。
